How to get started
- Initial consultation call: After we receive your information, a case manager will contact you within 12 hours to provide over-the-phone assistance and to schedule an assessment in person at home or in a hospital where you or your loved one is currently/planning to stay.
- In-home/hospital assessment: We’ll discuss your or your loved one’s needs and review all necessary medical documents and notes and determine the level of care needed moving forward; we will tailor a home health care plan for you or a loved one.
- Assigning caregiver & case manager: We will assign a caregiver for you or your loved one and as well as a dedicated case manager to provide an extra layer of service assurance and provide assistance whenever needed.
- Beginning of care: Once care is started, we will constantly monitor and periodically re-asses the care plan to determine if we need to adjust any of the services offered by the caregiver.
